You may donate directly to the Myasthenia Gravis Foundation of Arizona. Donations made directly to the foundation are tax deductable and enable us to continue offering programs and assistance to the many MG patients in Arizona. If you would like to make a donation or become a member please fill out one of our Membership forms and mail to us with your tax deductable donation.

Our Membership Form requires Adobe Acrobat to open and print it. If you do not have Acrobat Reader please visit their website to obtain this free program.
United Way
Combined Federal Campaigns

Fundraising campaigns for United Way and the National Voluntary Health Agency occur in many places of employement. If you make donations through an employer as a payroll deduction, your donation can be designated to go the Arizona Chapter of the Myasthenia Gravis Foundation.

If you or your family members are federal employees, please remember us in your Combined Federal Campaign through the National Voluntary Health Agency. The CFC code for Federal Government Employees is 0537 (this includes military).

If you are not a federal employee and would like to be a donor through the regular United Way Campaigns run through the state, our donor code is 2021. For further information, please call or speak with the Human Resources department of your company. A special thank you in advance for remembering us!
